This year is sure to be an awesome summer! We have new programs and teachers that are sure to keep you creatively occupied for the month of July!

Registration Instructions
CityArts Summer Arts Camp registration must take place in person during the registration dates, June 12th – July 2nd. Registration may be done Monday-Friday from 9am-4:30pm. There will be two special Saturday Registration Days June 12 & June 19 from 11am-2pm. Registration cannot be done over phone, email, or fax at this time, however if you have any questions about CityArts do not hesitate to call.

REGISTRATION MUST BE DONE IN PERSON. NO EXCEPTIONS
Eligibility Requirements:
Students must be between the ages of 8-14 years old as of July 6, 2010
Student MUST be a Providence resident

Registration Forms:
Spanish or English versions available in person or online (see below)
Course Request Slip (available online or at CityArts):
Students are required to list the classes they wish to take in order of preference. If form is not turned in, classes will be assigned at the discretion of CityArts Staff.
(One) Proof of Age Document:
Documents accepted: Birth Certificate (foreign accepted), Permanent Resident Card, or passport
(One) Proof of Address Document:
Documents accepted: Utility/phone bill in parent/guardian’s name, credit card statement in parent/guardian’s name, Department of Human Services correspondence in parent/guardian’s name.
*Child will not be registered until all necessary copies and forms are turned in.

You may print and fill out the forms at your convenience before you arrive at CityArts. Forms take about ten to fifteen minutes to complete so we suggest this option if time is an issue.

DIRECTIONS to CityArts:
Take 95 North or South to exit 18 (Thurbers Ave)
and follow signs for Eddy St. Continue straight
onto Thurbers Ave then turn right onto Broad St.
CityArts is 3 blocks down on the left side of the street.
Take RIPTA Bus #11 down Broad St
to Lexington Ave if going south
or Houston St if going north.
